Behaviour Change

MAP of Health Behaviour Change – This training programme is suitable for all registered healthcare professionals who have the opportunity to support people to make changes to their behaviour which will improve their health and wellbeing. It can support you to motivate someone to change, help someone plan and implement change and to maintain long-term change. ​​

 

Physical Activity

The DUK ‘Helping people with diabetes get active’ booklet provides practical solutions to support people to be as active as possible

Moving Medicine will provide you with an ultimate step-by-step guide to having conversations with patients about physical activity. Created in partnership with professional organisations, disease experts and patients, all handily packaged into structured conversations based on the time that you have available.

Brief advice from a health professional such as ‘giving permission to be more active’ can be effective. Motivate2Move offers useful education material to support discussion around increasing physical activity which are intended for use by practicing professionals.

 

Weight Management

Language Matters is designed to equip you with the skills to build your confidence when it comes to raising the issue of weight management with your patients with Type 2 Diabetes and referring them on to a weight management programme.
